Invitation for Tenders – HSIF Summative Evaluation

This document is to invite Evaluation expertise, consultants and agencies to tender for a contract to coordinate the evaluation of our Health Services Integration Fund (HSIF) initiative.

Contractors must complete a copy of the tender document by post and another copy via e-mail to the addresses shown below, demonstrating how they will meet the requirements.

The submission deadline for this tender is noon on Friday January 29, 2016 by which time all questions regarding some or all aspects of the tender should be submitted/directed to:

Contractors should note that the Lac La Ronge Indian Band Health Services and its partners will award the contract on February 15, 2016.

After the contract has been awarded, the successful contractor will begin on March 1, 2016.

The deadline for completion of this evaluation will be June 30, 2016.

The contract duration will cover complete evaluation of the La Ronge HSIF activities by planning, collection of relevant data, analysis and presentation/submission of final reports with possible recommendations.

The Lac La Ronge Indian Band Health Services (LLRIBHS) and the HSIF partnership reserve the right to clarify with any or all contractor(s) concerning any aspects arising from this Invitation to Tender after the tender submissions have been received.

Purpose

The purpose of this invitation is to provide the HSIF partnership with an overview of how collaboration can be enhanced within the three (3) partners to identify gaps, lapses, barriers and enablers to primary health care delivery on and off reserve. The evaluation will also serve as a mean of soliciting input and suggestions to improve collaboration on primary health care services and programs in La Ronge and area. The contractor may consider consulting with key stakeholders such as partners, LLRIB members, working groups, HSIF project team, Health Canada, among others. The objective of the evaluation will be to assess progress of HSIF activities by the partnership.

Contractor will be responsible for the full design and implementation of the evaluation process with support and resources provided by the partnership.
