Update for February 11, 2015:

To apply for Personal Credits on behalf of a former student who is either deceased or an incapable person, you must be a legally recognized representative of that person. You must provide proof of your authority to act as a Personal Representative of the CEP Recipient.

Please provide a certified true copy of the legal documentation that allows you to represent the former student (e.g. Power of Attorney, Non-Probated Will, Letters of Administration, Letters of Probate, Probated Will).  The legal documentation should clearly state, in detail, what powers the representative has and how they are to be executed.

Identification documents are also required and must also follow certain criteria to be acceptable.
